To ensure that you receive your reward, please go through the below steps you need to take and terms and conditions.
Steps you need to take
- Refer a friend using the form below. Be sure to have their mobile # or email address.
- Then to share a referral “CLICK” SEND AN INVITE to send a link to your friend via email/text.
- Ask your friend to mention your name while booking appointment/during their first visit.
- After your friend completes his/her first appointment, you will receive a $25 credit to your account.
- It's that simple! Now go refer a friend and get rewarded.
Referral Form
Fill the form with your your friend's contact information, valid phone #/email is requisite. Please provide your full name and contact information to receive credit for your referral.
Terms and conditions
- A 'Referral' is someone who is NOT an existing patient at Horizon Dental Care.
- The referral must book an appointment directly with Horizon Dental Care.
- The referral MUST reference your name at the time of registration.
- Once the referral completes his/her first appointment, the reward will be credited in respective accounts.
- The referral credit can be applied to dental treatment only and has no cash value.
- The rewards are non-transferrable. Must be 18 years or older to receive the rewards.
- Federal law prohibits Horizon Dental Care from offering this promotion to anyone who is receiving services as beneficiaries under Medicare or state health programs.
- The refer-a-friend program may end at any time.
